Kinds of Forms

Patients will bring forms for completion. They may present the form to you during the visit. They may drop off the form at the front desk during a time when they do not have a visit with you.

Providers are expected to complete forms within [#] business days.

• There are innumerable forms that physicians are expected to fill out.

• Below are among the most commons one we see with any specific information relevant to them.

• If there is a form that you have never seen before, or are unsure about, ask a preceptor or your box buddy.

• Be aware that some forms may require an attending's signature and NPI number.
